Associate Professor LEE Kheng Hock set up the first hospital-based family medicine department in Singapore in 2006. He is working towards creating a sustainable healthcare system for a rapidly ageing population. His work is validated by two landmark randomised control studies that proved the effectiveness of transitional care services in reducing unnecessary hospital utilisation.

In 2011, he was also appointed as the Medical Director of Bright Vision Hospital and had led the team to transform the hospital into Singapore’s first dedicated COVID-19 community isolation facility.
Assoc Prof Lee also teaches Family Medicine at the Duke-NUS Medical School Singapore. He incorporates patient-centred care and care integration into the Family Medicine curriculum. In 2022, Assoc Prof Lee was appointed as the Deputy Chief Executive Officer (Education and Community Partnerships), SingHealth Community Hospitals and he currently oversees the clinical training of three community hospitals.

Mr SNG Hock Lin leads the transformation of ActiveSG to support the organization’s Vision 2030 – to inspire the Singaporean Spirit and serve the community through sport. He was a senior army officer who participated in large-scale operations with multilevel stakeholders and often in complex and dynamic environments. He was also involved in organizational change management. A firm believer in creating value at work, Hock Lin excels in bringing out the best in people through engagement and development.

Currently, he is a PhD candidate in Gerontology, and his research interests lie in active ageing, exercise adherence and caregiving. As an Associate Faculty with SUSS, he facilitates modules on leadership and organizational transformation of non-profit organizations. Recently, he co-authored a book titled “Management and Leadership of Non-Profit Organizations in Singapore” to illuminate aspects of management challenges faced by non-profit organizations in Singapore.

Mr Adrian TAN has over 11 years of experience in procurement as well as logistics operations, and held key roles in continuous improvement and organization-wide change management before co-founding SG Assist, with a mission to build a sustainable ecosystem of care centred around the community and promulgate the "kampong spirit" through mobile technology.

SG Assist has been featured in various news and radio channels such as Channel 8 News and CNA on how the platforms have supported vulnerable communities, caregivers and social organizations during COVID, keeping the spirit of volunteering among Singaporeans.

A strong supporter and believer of second chances, he is also the Event Director for Help-Portrait Singapore – a social movement that provides free photography services for VWOs/Social Organisations such as senior activity centres, hospice care and women shelters to help them capture cherished memories.

Associate Professor Carol MA Hok Ka is known among academia and the community sector as an active and passionate practitioner of Service-Learning (S-L) and aging in Asia.

Currently, Carol is the senior fellow of the Centre for Experiential learning, associate professor, and Head of Gerontology Programmes (Master & PhD) at Singapore University of Social Sciences. She is leading the curriculum development, community engagement, and research in gerontology and promoting transdisciplinary education to address aging issues. She has led various aging and service-learning projects, namely, the elder learning development project, the age-friendly cities project (accredited by WHO), and the intergenerational project on health & wellbeing, etc. In view of the pandemic, Carol has taken the lead to work on a project on Reminiscences in Dementia Care during the COVID-19 pandemic with National Archives of Singapore and Dementia Singapore.
