Jockey Club Pathway to Healthy Ageing: A Nurse-led Response to United Nations Decade of Healthy Ageing 2021-2030
Abstract
The HKU School of Nursing is leading the Jockey Club Pathway to Healthy Ageing project in 8 Hong Kong districts. The project is a response to the declaration of the United Nation for the Decade of Healthy Aging 2021-2030. The project involves conducting a territory-wide healthy aging diagnostics using the WHO-ICOPE model. Older adults with accelerated aging are referred to receive an integrated care mapping, comprising person-centered care prescription by the ICOPE-based critical pathway, empowerment-based behavioral activation and professional-peer joint case management. By sharing the preliminary impact, the presentation sheds light on developing a sustainable healthy aging model in HK.
